DIVERMANSION PINECREST CORP – WAIVER AND RELEASE FORM
Use of Facilities – Assumption of Risks and Release of Liability
1. Assumption of Risk: I understand that using the facilities carries inherent risks that can cause injuries or damage. I agree to participate voluntarily and to assume all responsibility for myself and for the minors under my care.
2. Compliance with Rules: I commit to following all the rules and policies of the facility. DIVERMANSION PINECREST CORP reserves the right to deny entry or ask a person to leave for non-compliance.
3. Release of Liability: I hereby release and hold harmless DIVERMANSION PINECREST CORP, its employees, owners, and representatives from any and all liability for any accident, loss, or damage that may occur during our visit, even if caused by negligence.
4. Responsibility for Damages: I agree to pay for any damage caused by me or by the minors under my care through intentional or negligent actions.
5. Health and Safety: I declare that neither I nor the minors in my care have any symptoms of illness or conditions that could put others’ safety at risk. I accept responsibility for any medical expenses that may arise.
6. Specific Safety Rules:
- Wearing socks inside the play area is mandatory.
- An adult must accompany children at all times.
- No food, drinks, or gum are allowed in the play areas.
- Climbing on the outside of the structures or wearing clothing with drawstrings or hoods is not allowed.
- Report any inappropriate behavior to staff.
- DIVERMANSION PINECREST CORP is not responsible for lost or stolen items.
7. COVID-19: I acknowledge that there is a risk of exposure to COVID-19. I declare that I have no symptoms and have not been recently exposed to the virus, and that I am following the current health recommendations.
8. Dispute Resolution: Any dispute will first be attempted to be resolved through mediation, in accordance with the laws of the State of Florida.
9. Images and Promotion: I authorize DIVERMANSION PINECREST CORP to use photographs or videos in which my child appears for promotional purposes. This authorization can be revoked in writing.
By signing this document, I declare that I have read, understood, and accepted this agreement in its entirety, and that I sign it voluntarily on behalf of myself and the minors in my care.
